Icon Design and Development
The attention to detail that can be found by concentrating on the smaller aspects of a project, most certainly contribute to the overall picture.
Ignore them at your peril. Some people do, I however believe that iconic design is as important as any part of the complete website.
Although I do design my own icons, I also believe in not reinventing the wheel. There are plenty of great resources out there from dedicated icon shoppes where you can almost certainly integrate seamlessly into your projects. Of course customisation is available too.
